Workers Comp Incident Reporting

We require that all employees report job-related accidents or injuries to a manager immediately, whether the accident occurred on or off company premises. Failure to report an injury, regardless of how minor, could result in difficulty with the employee’s claim. All workers’ compensation claims will be paid directly to employees and employees are expected to return to work immediately upon release by their doctor. Additionally, you need to fill out one of the following bar-specific forms that will provide the details we need to know about your injury.

Maintenance Issue Reporting

In order for us to be able to fix maintenance issues quickly, we need staff to report them in a timely manner so critical issues can be addressed quickly before the start of business the next day.  Please use the one of the following bar-specific forms and our maintenance staff and managers/owners will be notified immediately.

Customer/Security Issue/Incident Reporting

We are committed to dealing with customer/security incidents/issues in an ongoing basis. Please report customer/security incidents/issues so that we can review them and reach a decision on how they should be handled. You can either print out the Incident Report here, and upload the filled out report using the bar-specific form below, or you can go directly to the bar-specific form below and fill out a simplified version online to alert managers/owners to a customer/security issue/incident.

We are maintaining an online customer 86’ed list.  Please use the following Add To Customer 86’ed List link to make a request to add a customer to the 86’ed list.  All bars will be notify of an addition to this list.

Injury and Illness Prevention Program

The full Injury and Illness Prevention Program can be found here.  It is this company’s policy to maintain open communication between management and staff on matters pertaining to safety.  All input regarding safety is considered important, and employees are encouraged to actively participate in the company safety program.  Employees should feel free to express any safety concerns during safety meetings, staff meetings, individually to managers or in writing.  All safety suggestions will be given serious consideration and each will receive a response.  To facilitate the reporting of any safety issues/concerns, please use this the bar-specific form link below..  If you prefer to report anonymously, that’s OK as well.  You don’t need to fill in your name on the form.

OSHA requires that all establishments covered by CCRTitle 8 Section 14300 must complete the Cal/OSHA Form 300A Annual Summary of Work-Related Injuries and Illnesses, even if no work-related injuries or illnesses occurred during the year.  These are posted in each bar office.  For your convenience, we’ve provided them here as well:
